• Conduct the Initial assessment of the patient and home situation and determination that the patient's status and care needs can be safely met by homemaker-home health aide service
• Verify the patient's eligibility and appropriateness for admission based on agency policies and regulations.
• Referral of the patient at any time to a home health care agency or other appropriate level of care, when the patient's status and care needs require more than supportive services
• Development and periodic review of a written plan of care which shall include the frequency of assessment and methods by which the patient's status and care needs are to be monitored between assessment visits in the home.
• Coordinate and oversee services rendered to the patient and their family.
• Develop and update written instructions for the homemaker-home health aide in alignment with the plan of care.
• Orient the homemaker-home health aide in the home, to the patient, family and plan of care;
• Evaluate the competency of homemaker-home health aides to carry out all assigned patient care activities.
• Plan and document RN supervision of homemaker-home health aides, ensuring supervision includes direct observation of care in the home.
• Arrange for supervision of homemaker-home health aides by physical therapists, occupational therapists, speech therapists, or social workers, when appropriate.
• Develop a plan for medical or other emergencies in collaboration with the care team and patient.
• Maintain ongoing communication with the patient’s source(s) of medical care to ensure appropriate interventions when necessary.
• Develop, maintain, and ensure the accuracy of the patient care record, in compliance with agency policies and state regulations.
• Participate in the orientation and in-service education programs for homemaker-home health aides.
• Review records and reports prepared by the homemaker-home health aides to monitor compliance and care quality.
• Conduct case management conferences with the homemaker-home health aides
• Provide written performance evaluation of homemaker-home health aides
• Consult with the agency administrator on all aspects of patient care, ensuring alignment with agency policies and procedures
